The license/registration issued
to you by the Division is public proof that you are entitled to pursue certain
activities regulated by Idaho Law.  If your license/registration becomes
lost or destroyed, you may
print a new copy through your Online Services account.  

MASSAGE FEES

Application Fee: $50

Initial License Fee – Endorsement: $150

Initial License Fee – Examination: $130

Provisional Permit Fee: $25

Renewal Fee for birthdates ending in an odd year: $130 for two (2) years

Renewal Fee for birthdates ending in even-numbered year: $65 for one (1) year

Reinstatement Fee: $35

Certified license history: $10

 

A provisional permittee pays the $50 application fee and $25 permit fee (total of $75).  After passing the exam, the applicant must upgrade to the full massage license application and pay the $130 initial license fee for a two-year license.  There is no application fee for the upgrade.
